Official guidelines for preparing, assembling and submitting the Industrial Training report and logbook — covering document order, reporting-form codes, the weekly logbook, report content and formatting rules.

Critical priority

Submission requirements

Strict compliance is required. Non-compliance will adversely affect your overall marks for the general internship assessment.

Read this before you submit

The logbook signed by the industrial supervisor, the report and the accompanying slides must be consolidated into a singular PDF file. Any handwritten content must be diligently scanned and incorporated into that comprehensive report.

  • One single file only. The signed logbook, the report and the internship slides must be merged into one PDF.
  • Scan all handwritten content. Signatures, handwritten logs and approvals must be scanned clearly and inserted at the correct place in the document.
  • Marking impact. Late or non-compliant submissions adversely affect your overall marks for the general internship assessment.

File naming convention

Rename the consolidated PDF exactly as follows.

Double-check the name and ID

Write your full name as registered and your student ID without spaces. An incorrectly named file may not be matched to your submission record.

On the Industrial Training Reporting Form you must tick (X) the correct code for your placement. Use the search box to locate your field quickly.

Reading codes 7 and 8

In the printed form the wording of codes 7 and 8 is split mid-sentence. Read them together: code 7 covers wholesale and retail trade plus the repair of motor vehicles, motorcycles and personal / household goods; code 8 covers hotels and restaurants. Tick the code that matches your host company’s main business.


Part B — Industry code (organisation type)

Tick one code from 1 to 8 to identify the type of organisation where you are placed.

Company Description (page 7)

The Company Description section must state both the Economic Sector (Part A) and the Industry Code (Part B), followed by a short written description of the organisation — what it does, where it operates and how your department fits into its operations.

Section 05

Statement of absence

Compulsory for any student who is absent during the Industrial Training period. Complete it at pages 8–9, immediately after the Company Description.

No proof, no approval = illegal absence = subject to termination by the company

It is COMPULSORY for students to fill in this form if they are absent during the Industrial Training period. The student must obtain approval from the Industrial Supervisor, and must also provide a copy of their reason for absence as proof.

How to complete the form

  • Record every day of absence — one line per date.
  • State the reason clearly (medical leave, compassionate leave, emergency, and so on).
  • Enter the number of day(s) taken for that entry.
  • Obtain the industrial supervisor’s approval in the Approved By column.
  • Attach the original proof (medical certificate, letter, etc.) as a scanned copy immediately behind the form.
  • Total the days in the TOTAL DAYS row before submission.

The template provides six entry lines plus a TOTAL DAYS row. If you have more absences, duplicate the form on the next page and continue the numbering.

If you were never absent

Keep the section in the document exactly as issued. Do not delete the page just because you had no absences — the fixed document order must be maintained.

Section 06

Logbook

The logbook occupies item 5 of the consolidated document (from page 10). It is written weekly and verified by your industrial supervisor as you go — not retrospectively.

What to write

  • Use past tense and be specific about the tasks you personally performed.
  • Name the equipment, software, standards or documents involved.
  • Note problems encountered and how they were resolved.
  • Keep each entry to a short, readable paragraph — one week per block.
  • Do not copy the same paragraph week after week; entries must show progression.

Sign-off rules

  • Every week must be checked by the industrial supervisor.
  • Digital signatures are permitted in the Checked By column.
  • If the supervisor signs by hand, scan the page and insert the scan into the consolidated PDF.
  • Weekly sign-off must happen during the placement week, not all at the end.
Number of weekly blocks

The template in the handbook provides repeated weekly blocks covering a full placement period (approximately 19 weeks). Continue the blocks from the template for the whole duration of your training — do not stop at the number of blocks shown as an example.

Section 07

Report & internship slides

The report begins at page 30 and the internship slides close the document at page 34. Follow the prescribed outline exactly — do not rename or merge chapters.

Report outline

Each numbered item below is a required part of the report. The indented items are sub-sections of the part above them.

  1. Abstract

    A concise summary of the training placement, the work carried out and the main outcomes.

  2. Introduction

    Opens the report and contains the following sub-sections.

    • Historical Background — overview of the company’s origins and development within the industry.
    • Main Activities Structure — description of the primary operational functions and organizational framework.
    • Objectives of Practical Training — outline of the key skills and competencies intended to be acquired during the training period.
    • Scope of Training / Schedule of Training — detailed plan outlining the duration and sequence of training activities across the internship term.
    • Company Profile — comprehensive overview of the organization, including its mission, size, and operational scope.
  3. Industrial Environment

    Covers how the workplace operates and how you adapted to it.

    • Adaptation and Adjustment to The Industry
    • Practice of Occupational Safety and Health Environment
  4. Industrial Experiences

    The tasks, duties and responsibilities you carried out across the placement.

  5. Industrial Projects

    The project(s) you were assigned to — objectives, your contribution, methods, results and challenges.

  6. Summary

    Overall reflection on the training and the competencies gained.

  7. References

    All sources cited in the report, formatted in IEEE style.

  8. Appendix

    Supporting material: drawings, data sheets, photographs, letters, extra logs.

  9. Internship Slides Must attach

    Your presentation slides are attached at the end of the report as part of the same consolidated PDF.


Memo on consolidation

Logbook + report + slides = one PDF

It is crucial that the logbook signed by the Industrial Supervisor, the report and the accompanying slides be consolidated into a singular PDF file. Any handwritten content must be diligently scanned and incorporated into this comprehensive report. Non-compliance with these instructions will adversely impact your overall marks for the general internship assessment.

Rename the file

Section 08

Document formatting rules

Apply these instructions consistently to the whole document. Markers deduct for inconsistent fonts, missing captions and an absent table of contents.

1. General formatting

  • Font: use Arial for all text in the document.
  • Header size: Arial, 20 pt, bold.
  • Body text size: Arial, 12 pt.
  • Alignment: centre-align all general information except main headings.

2. Headings

  • Heading levels: use headings to organise the document into clear sections and subsections.
  • Main headings: Arial, 20 pt, bold.
  • Subheadings: use descending font sizes and styles as needed — for example Arial 18 pt bold for first-level subheadings and Arial 16 pt bold for second-level subheadings.

3. Table of contents

Your document must include a Table of Contents for easy navigation. Build it from the document headings so the entries and page numbers stay accurate.

4. Figures and tables

  • Captions: every figure and table must have a caption.
  • Figure captions: place the caption below the figure.
  • Table captions: place the caption above the table.
  • Numbering: number all figures and tables consecutively (Figure 1, Figure 2, Table 1, Table 2 …).

5. Margins and spacing

  • Use standard margins (1 inch on all sides).
  • Use single or 1.5 line spacing for body text.

6. References

Follow the appropriate citation style; use IEEE style. Example of an IEEE-formatted journal article:

Before exporting to PDF

Update the Table of Contents field (it should not show “Error! Bookmark not defined” or placeholder text), then export the whole document — logbook, report and slides — as a single PDF.

Supervisor evaluation forms

Two evaluation forms complete the consolidated document: one from your industrial supervisor and one from the University’s Main Supervisor.

Industrial supervisor’s evaluation

Completed and signed by your company supervisor at the end of the training placement. Arrange this early — the form must be signed before you finalise the PDF.

Lecturer’s formal evaluation report summary
